What information is required to complete the registration form?

PLEASE READ THIS PAGE CAREFULLY BEFORE COMMENCING YOUR CHILD'S REGISTRATION

Digital copies of documents (images or scans) may be required to complete the Registration for the First Holy Communion 2024 including the First Reconciliation Programme. Please read the "Information About This Form" below to be prepared and have digital copies of documents ready before commencing the registration. As an alternate to uploading documents, you may email the documents to (remove XX) sacraments.mudgee XX @cdob XX .org.au.

Page 2 - Information Required About Your Child Your child's complete name (first, middle and last names), birthdate, school, school grade, sacraments completed at church and location (Baptism). A copy of your child's Baptismal Certificate may be required if not completed in the Catholic Parish of St Mary of the Presentation Mudgee (St Mary' of the Presentation Mudgee, St John the Baptist Gulgong, St Dominic's Kandos, St Malachy's Rylstone).

Page 4 - Mother's Required Information Relationship to child, mother's/carer's full name, mother's maiden name, mother's email address, mother's phone number. If circumstances exist that may prevent both parents/carers providing information then you may confidentially discuss this matter further before completing this form. BOTH parent's/carer's names and check box acknowledgements are required. If circumstances exist that may prevent both parents/carers giving consent for your child's participation in the First Holy Communion including First Reconciliation 2024 programmes then you may confidentially discuss this matter further before completing this form. Page 7 - General Film and Photography Consent Check box acknowledgements of your Consent for your child to be Photographed while participating in the First Holy Communion including First Reconciliation 2024 programmes. BOTH parent's/carer's names and check box acknowledgements are required. If circumstances exist that may prevent both parents/carers giving consent for your child to be Photographed while participating in the First Holy Communion including First Reconciliation 2024 programmes then you may confidentially discuss this matter further before completing this form. Page 8 - Parent Information Session Booking You must book one of the 2 required Parent Information sessions being held in July 2024. Attendance at one of the two scheduled Parent Information Sessions is a requirement to the programme. If for any reason you can not attend either of the two Parent Information Sessions then please inform the Sacramental Team in writing via email at (remove XX) sacraments.mudgee XX @cdob XX .org.au.

Page 9 - Sacrament Contribution and Payment Choose to pay your contribution by direct bank deposit or in person at the Parish Office. If by direct deposit, either upload a copy of your Bank Deposit Receipt or email a copy of your Bank Deposit Receipt to (remove XX) sacraments.mudgee XX @cdob XX .org.au The contribution for the Sacrament of First Holy Communion including First Reconciliation 2024 programmes is $70 per child. Where more than one child in one family is participating in the Sacrament of First Holy Communion including First Reconciliation 2024 programmes, then please contact the Parish Office on 02 6372 2122 for family fee arrangements.

No child will be excluded from participating in any of our Sacramental Programmes due to any financial concerns. Should you wish to confidentially discuss this matter further, please contact the Parish Office.

What can we do if we can't attend either of the scheduled First Holy Communion Masses?

Your child's First Holy Communion Masses will be celebrated by our Priest on Saturday 26th October 2024 and Sunday 27th October 2024. Any additional dates will be considered subject to the demand of requests in writing by parents/carers requiring an alternate date for their child's First Holy Communion Mass. As soon as you know that you are unable to attend either of the above Masses then you must immediately notify the Sacramental Team by email so that alternative arrangements may be planned. 


What do I do if I can't upload my registration documents?

We have noticed that some parents may not be able to upload documents during the registration process. If that happens to you then attach your documents to an email and send to (remove XX) sacraments.mudgee XX @cdob XX .org.au.


What clothing should my child wear on the day of the Sacrament of First Holy Communion?

In reverence and respect for the significance of the Sacrament of First Holy Communion we ask that children wear their "Sunday best clothes" during the Mass. Also be aware that the parish provides smocks that the children must wear over their clothes for the entire Mass.


What happens on the day of the Sacrament of First Holy Communion?

We would like your child to meet in the Parish Hall 40 minutes prior to your Saturday and Sunday booked Masses. We will be instructing the children about their part in the Mass and helping them to put on a smock. A parent may stay to assist. The children will be processing into the church ahead of the Priest.

Parents and guests should be seated in their allocated pews well before the Mass commences. The family's name will be at the end of the pew.
