Tip: If using a smartphone, you can choose "Take Photo" directly during the upload step of the form.

Step 1: Candidate Information

Enter your child's full legal name as it appears on their Baptism certificate. Enter your child's birthdate. If your child’s school is not in the list, select "Other" to type the name manually and indicate if it is a Catholic school. Enter your child's school grade.

 Previous Sacraments

Provide the details of where your child received their previous sacraments. Use the "Choose File" buttons to upload the certificates if required.

Step 2: Parent Information

Enter the contact details for Parent 1 (the primary contact) and Parent 2.  For Parent 2, you may click the button "same Address as P1" to copy the Parent 1 street address.

If circumstances exist that may prevent both parents/carers providing information including participation consent and photography consent then you may confidentially discuss this matter further before completing this form. Please contact the Parish Office on 02 6372 2122.

Single Parent Households: We recognize that every family’s circumstances are unique. By identifying as a single parent or sole guardian, you confirm that you have the legal authority to provide consent for your child’s registration. This helps us ensure that all pastoral and legal requirements are met with care and respect for your family's situation.

 Family Information for Pastoral Preparation

To help our parish priest provide the best possible pastoral support for your family's sacramental journey, we ask a few questions about your family structure. This information is confidential and accessible only to the parish priest. It will not affect your child's eligibility for any sacrament. Providing this information is voluntary.

Step 3: Parent Information Session

Attendance at a Parent Information Session is a mandatory part of our sacramental journey. Please select your preferred date from the dropdown menu.

Step 4: Contribution

The suggested contribution amount will be displayed based on the program. Choose your preferred payment method:

  • BPoint: Pay immediately via secure Credit/Debit card link.
  • EFT: Direct deposit via your banking app using your Reference Code.
  • Office: Visit the Parish office during opening hours.
  • Family: Only for a 2nd registration from the one family household.
